Program Details

The Big Bend Rural Health Network is one of nine established in the state of Florida per Section 381.0406, Florida Statutes.  We collaborate with others in the region to ensure that our residents can improve or prevent chronic diseases.  Our staff members are trained in chronic disease self-management, diabetes, wellness, and tobacco dependence.

Our Master-prepared staff members are trained in chronic disease self-management, diabetes, wellness, and tobacco.   In addition, in the past 10 years, we have assisted in the training of more than 40 CHWs through our partnership with Big Bend Area Health Education Center.  Our (community health worker) CHWs are trained to provide health information, implement programs, and make referrals to health resources.  Since 2014, we have Franklin, Gadsden, Jefferson, Madison, Taylor, Wakulla, and Leon Counties Big Bend Rural Health Network (BBRHN) is dedicated to eliminating health disparities in our minority, rural populations.  For almost 25 years, BBRHN has implemented programs in successful trained CHWs to meet program demands.
